Frequently asked questions
OnTrack Lifestyle Medical is not a replacement for your traditional primary care provider. Your PCP is an essential partner in your health—they provide urgent and after-hours coverage, manage routine sick visits, and serve as your gateway to specialty care.
At OnTrack, we complement this role by offering:
Concierge-level time and attention for prevention, lifestyle change, and health optimization.
Continuity and coordination of care across your specialists, ensuring your health story is organized and up to date.
Guidance, coaching, and oversight to help you stay on track with your goals and get the most from your care team.
A collaborative partnership with your PCP and specialists, with more strategy, more support, and more time devoted to you.
With our lower patient numbers and personalized approach, we are able to support the many facets of your health and ensure nothing falls through the cracks.
No. OnTrack Medical is a membership-based practice and does not bill insurance. This allows us to provide unrestricted time, attention, and flexibility without the limitations of insurance-driven care.
You can still use your insurance for:
· Lab work
· Imaging
· Medications
· Specialist visits
· Hospitalizations
Our role is to coordinate, interpret, and integrate those results and visits into your personalized plan.
OnTrack Lifestyle Medical does not provide urgent care or treatment for acute issues. If you have an urgent or emergency concern, please contact your primary care provider, visit an urgent care center, or go to the emergency room.
Our role begins once the immediate issue is addressed. We support you by:
Helping interpret results and recommendations.
Assisting in medical decision-making and advocacy.
Coordinating follow-up care with your providers.
Helping prevent hospital readmissions through close monitoring and proactive support.
Ensuring your long-term health plan stays on track.
To establish care, we require an in-person comprehensive physical as part of your Benchmark Assessment.
After that, your ongoing care is tailored to your needs:
Most support is provided virtually for flexibility and convenience.
In-person visits are available whenever needed or preferred.
You will also have direct text messaging access to your provider.
Secure portal access is available to stay connected and organized.
This blended approach ensures you have continuous support that’s accessible, personal, and aligned with your lifestyle.
What is Lifestyle Medicine? Lifestyle Medicine is a medical specialty that uses evidence-based lifestyle interventions to prevent, manage, and even reverse many chronic diseases. Rather than only treating symptoms, it focuses on addressing the root causes of illness to improve overall health, reduce reliance on medications, and enhance quality of life.
At its core are the six pillars of health:
Whole-food, plant-forward nutrition
Regular physical activity
Restorative sleep
Stress management
Positive social connections
Reduction or elimination of risky substances
By building care around these six pillars, Lifestyle Medicine empowers patients to take meaningful steps toward a healthier, more balanced, and fulfilling life.
Yes. Both the Benchmark Assessment and the Comprehensive Medical Review can be completed as stand-alone services. After your assessment, you’ll receive a personalized health roadmap (or Medical CV, depending on the option you choose). While many patients decide to continue with a membership for ongoing support and continuity of care, there is no obligation to enroll.
