Benchmark Assessment

Best for individuals focused on prevention, performance, and optimization.The Benchmark Assessment is your personalized starting point for understanding your health, identifying potential risks, and building a roadmap for long-term wellness.

It includes:

  • Medical history & Physical exam: A 1-hour, no rush, comprehensive physical examination with a full review of your medical records beforehand.
     

  • Follow up assessment: A 1-hour follow-up session virtual or in person after advanced testing is completed to review results and insights.
     

  • Publishing of medical CV: A personalized health roadmap with targeted strategies for nutrition, fitness, sleep, hormone balance, and disease prevention.
     

  • Eligibility for membership: Optional continuation with a Core or Momentum Membership

Comprehensive Medical Review

Best for patients or caregivers managing complex medical histories.The Comprehensive Medical Review provides an in-depth evaluation of your complete health history, helping bring clarity and coordination to complex care.
It includes:

  • Medical records review: Extensive physician time dedicated to reviewing and synthesizing your full medical records.
     

  • Publishing of medical CV: A master document that organizes your diagnoses, treatments, lab and imaging results, medications, allergies, and key health events into one clear, portable file.
     

  • Follow up assessment: Physician insights that highlight gaps, patterns, priorities, and actionable recommendations in your care.
     

  • Oversight coordination: One time coordination with care team to review findings and plan. 
     

  • Eligibility for membership: Optional continuation with a Navigator Membership, which includes ongoing updates to your Medical CV and active care advocacy.

After completing your Onboarding Assessment, you’ll become eligible to join one of our Membership Services. These memberships provide ongoing guidance, accountability, and personalized support to keep you on track with your health goals.

At OnTrack Medical, we create a Medical CV—a master document that brings your entire health history together in one clear, portable file.
